Admission - College of Dentistry

ADMISSION POLICIES OF THE COLLEGE OF DENTISTRY

Any student, regardless of nationality, culture or religious belief, shall be admitted in the College of Dentistry, provided that the student is willing to comply with school policies, rules and regulations.

Admission Requirements:

The applicant must:
a. take and pass the entrance examination given by the Guidance Office.
b. take the dexterity test conducted by the College.
c. submit to an interview by the Admission Committee.

a. For First Year (Pre-Dental Education)

The applicant must submit the following documents:
1. Application Form (from DMSF Dental Office) with application fee
2. Original Form 138 (High School Report Card) and a photocopy
3. NSO-authenticated copy of Birth Certificate and 3 photocopies
4. Recommendation Letter from High School Principal or Guidance Counselor and a photocopy
5. 2 copies of 2”x2” and 1”x1” ID Pictures (with red background)

b. For Third Year (Dental Proper)

The applicant must have:
b. Have completed a two-year pre-dental program or a degree to health in order to meet the requirements for a Certificate of Eligibility for Dentistry (CED) issued by CHED.
c. Have been issued a Certificate of Eligibility for Dentistry.
d. Submit the following documents:
1. Certified true copy of the Transcript of Records
2. Original copy of the Certificate of Honorable Dismissal
3. Certificate of Eligibility for Dentistry
4. Certificate of Good Moral Character from 2 members of the faculty
5. NSO-authenticated copy of Birth Certificate and 3 photocopies
6. 2 copies of 2”x2” and 1”x1” ID Pictures (with red background)

c. For Transferees

The applicant must:
a. take and pass the validating examination given by the College.
b. submit the following documents:
1. Certified true copy of the Transcript of Records
2. Original copy of the Certificate of Honorable Dismissal
3. Certificate of Good Moral Character from 2 members of the faculty
4. NSO-authenticated copy of Birth Certificate and 3 photocopies
5. 2 copies of 2”x2” and 1”x1” ID Pictures (with red background)

The Admissions Committee shall decide on the application for transfer of students from other dental schools on a case-to-case basis and shall not accept applications for transfer if the reasons include academic failures or dishonorable dismissal.

d. Foreign Applicants

Foreign applicants shall be subject to the same application and admission requirements. Upon acceptance, they must submit the following documents:
1. Special Study Permit (Student Visa)
2. Permit to study issued by the Commission on Higher Education (CHED)
3. A photocopy of the Student Visa
4. Alien Certificate of Registration
5. Immigrant Certificate of Residence

Student Visa application should be submitted at least six (6) months before the opening of the school term (schools in the Philippines open in June for the first semester, November for the second semester, and April for the summer.

Requirements for the issuance of a student visa:
1. Original copy of DMSF’s Notice of Acceptance
2. Original copy of the student’s Personal History Statement (PHS) duly signed and with 2”x2” photo and right hand thumbprint
3. Proof of adequate financial support to cover expenses incidental to the study
4. Authenticated scholastic records
5. Authenticated Police Clearance Certificate issued by the national police authorities in the student’s country of origin
6. Medical health certificate issued by an authorized medical practitioner in the student’s country of origin, including a standard size chest x-ray
