Admission Process:
- Submit to the respective Office of the Dean the above listed documents for assessment. Copies can be submitted by email or can be submitted directly to the Office of the Dean.

- If the requirements are complete, the applicant will be notified by email on the schedule of the interview or if documents are submitted directly, an interview can be done on the same day.
- A Letter of Acceptance (LOA) will be given to those who qualify for admission. The accepted applicant will be asked to pay the commitment fee for slot reservation. The following are the required commitment fees:
Biology – P1,000.00
Dentistry – P2,500.00
Midwifery-P500.00
Nursing – P1,000.00
Commitment fees will be deducted from the required downpayment upon enrolment. Commitment fee is non-refundable should the applicant decides not to proceed with enrolment.
Note: The applicant who intends to enroll right after receiving the LOA (on the same day) must inform the Admission’s Committee to claim the Pre-Registration Note (PRN) and will have to pay the whole amount required as downpayment upon enrollment. No need to pay for the commitment fee.
- Present the receipt of commitment fee to the Dean’s Office and request for the Pre-Registration Note (PRN). A PRN contains the list of subjects allowed to be enrolled. After receiving the PRN, the applicant may now commence the enrollment process.
- With a LOA and commitment fee payment, the applicant can also proceed to the School Clinic for the required laboratory test to be done at the DMSH Hospital.
Enrollment Instructions:
Note: The student is required to be present to do the enrollment process to fulfill the required enrolment procedures such as declaration and signing of personal information, sign the waiver and affidavit and to go for laboratory examination at the DMSF Hospital. Only those who have undergone the admission process, with a Letter of Acceptance (LOA), Pre-Registration Note (PRN) released by the respective College and a receipt of the paid commitment fee are allowed to enroll:
Step 1: Together with the above required original documents presented in the admission process, placed on the specific colored envelop, present the Letter of Acceptance (LOA), PRN and all original documents at Window 7 of the Registrar’s Office.
Step 2: Fill out the following:
⮞ Enrollment Form (Copy all subjects and units from your PRN)
⮞ Student Profile
⮞ Affidavit of Undertaking
⮞ and School Waiver
Step 3: Obtain the following signatures in sequence:
- Guidance Office – (fill out the Personal Data Sheet and interview with the Guidance Head)
- Office of the Student Affairs (OSA)
- School Clinic – (Obtain the request for laboratory test to be done at DMSF Hospital. Submit result at the School Clinic)
- School Cashier (payment for the downpayment and for the notarization of Affidavit of Undertaking)
The following are the options on how to pay for the downpayment and notarial fee:
- Payment at the DMSF Cashier (secure your queuing number)
- Over the counter in any of the following banks (Metrobank, RCBC and PNB)

For options 2 or 3, kindly print the successful bank transaction slip and show them to
Window 9 In-charge for checking and signature in the enrollment form. A copy of the official receipt of the downpayment can only be claimed after 5 days after payment.
First Semester Downpayment:
Dentistry: P9,500.00 + 200 (Notarial Fee) = P9,700.00
Nursing: P12,500.00 + 200 (Notarial Fee) = 12,700.00
Midwifery: P5,500.00 + 200 (Notarial Fee) = 5,700.00
Biology: P12, 500.00 + 200 (Notarial Fee) = 12,700.00

Step 4: Once you have acquired all the necessary signatures, kindly submit the fully accomplished Enrollment Form, Student Profile, Affidavit of Undertaking, and School Waiver at the Registrar’s Office Window 8 for signing of the enrollment form and printing of your Enrollment Registration Certificate (ERC).
Note:
A student is only considered officially enrolled if an Enrollment Registration Certificate (ERC) with a stamped “Officially Enrolled” is received from the Registrar’s Office
Kindly present your Enrollment Registration Certificate (ERC) to the Office of Student Affairs (OSA) for ID capturing.
ERC must be presented to the respective instructors on the first day of class. This will ensure that the student’s name is listed as enrolled in that subject.
New Student for College of Medicine
Requirements for Admission to the College of Medicine

- Certificate of the General Weighted Average in College (GWA)
- Photocopy of Diploma
- Photocopy of NMAT result
Admission Process of the College of Medicine: 1st Year Medicine
- Submit to the College of Medicine the above listed documents for assessment.
- If the requirements are complete, the applicant will be scheduled for a psychological exam. An application and entrance exam fee of P1,300.00 will be required.
- After the release of the entrance exam result, an interview with the Admission’s Committee will be done.
- A Letter of Acceptance (LOA) will be given to the accepted applicant. Payment of P10,000 commitment fee will be asked for slot reservation.
- After receiving the LOA and paying the commitment fee, the applicant may now commence the enrollment process.
- With a LOA and commitment fee payment, the applicant can now proceed to the School Clinic for the required laboratory test to be done at the DMSH Hospital.

- Original Copy of Transcript of Records (Baccalaureate/College)
- Transfer Credentials Certificate (Honorable Dismissal)
- Original Copy of National Medical Admission Test (NMAT) result
- Certified Copy of CHED Special Order (for those coming from non PAASCU accredited programs and other accreditation body)
- Original Copy of Birth Certificate from PSA (Philippine Statistics Authority) with Receipt
- Original Copy of Marriage Certificate from PSA (Philippine Statistics Authority)
with Receipt for Married Student
- Certificate of Good Moral Character from School Head/ Guidance Councilor
- Diploma (Authenticated by the School Registrar or Certified Copy only) or
Certificate of Graduation
- 2 pcs 2×2 Size ID Picture with Blue Background (White Shirt/Blouse w/ Collar)

Guide for Enrollment:
Note: Only those who have undergone the admission process and are given Letter of Acceptance and has paid the commitment fee are allowed to enroll.
Below are the steps for enrollment at the Registrar’s Office:
A. Present at the Window 6 of the Registrar’s Office all original required documents, Letter of Acceptance and receipt of the downpayment. Fill out the Enrollment Form, Student Profile and Affidavit of Undertaking.
B. Obtain the following signatures in sequence:
- Guidance Office (fill out the Personal Data Sheet and interview with the Guidance Head)
- Office of the Student Affairs (OSA)
- School Clinic (Obtain the request for laboratory test to be done at DMSF Hospital. Submit result at the School Clinic after)
- Cashier (payment for the required downpayment of P38,000.00 less the commitment fee and fee for the notarization of Affidavit of Undertaking)
The following are the options on how to pay for the downpayment:
a.) Payment at the DMSF Cashier (secure your queuing number)
b.) Over the counter in any of the following banks (Metrobank acct details, RCBC acct details, PNB acct details)

For options B or C, kindly print the successful bank transaction slip and show them to Window 9 In-charge (No need to get a queuing number) for checking and signature in the enrollment form. Issuance of official receipt will take 5 days after validation.

Once all the necessary signatures are acquired, submit the fully accomplished Enrollment Form, Student Profile and Affidavit of Undertaking at the Registrar’s Office Window 6 for printing of your Enrollment Registration Certificate (ERC).

Step 1: Initial Evaluation Documents
Foreign students seeking admission to the College of Medicine shall submit the following documents in the order outlined below:
Completed Application Form and Personal History Statement (can be downloaded in our website: www.dmsf.edu.ph)
Photocopy of National Medical Admission Test (NMAT) result
Letter of Reference from two (2) former professors
Application Fee (non-refundable)

Step 3: Initial Evaluation Results
Submitted documents in item A will be evaluated by the Office of the Dean, after which, the applicant is informed as to the status of his/her application. If applicant is qualified, the school will send to the applicant the following:
Notice of Acceptance (NOA)*
Personal History Statement forms for the applicant to fill-up (6 copies)
Certificate of Eligibility for Admission (CEA) form for the applicant to fill-up (not required for lateral entrants)
Letter of Reference from two (2) former professors
Application Fee (non-refundable)
*The Notice of Acceptance should not be construed as the actual enrollment.
Step 4: Submission of Documents to the Office of the Dean
Official transcript of records and diploma;
Notarized Affidavit of Support, including bank statements or notarized notice of grant for institutional scholars to cover applicant's expenses for school dues, accommodation, subsistence and other incidental expenses;
Photocopy of passport pages where name, photo, birthday, birthplace appear;
Students Personal History Statement (6 copies) with 2 x 2 recently taken photograph;
Police Clearance issued by the National Police Authorities in the student's country of origin or residence;
Birth certificate;
Letter of Recommendation from two former college professors;

DFA notifies the Foreign Service Post concerned on the issuance of a student visa. The applicants will now be required to appear in person before the Philippine Foreign
Service Post with consular jurisdiction over the place for interview.
Step 5: Student Visa
After receiving confirmation for all the previous steps, the following documents are required for the issuance of a Student Visa
Original copy of the school's Notice of Acceptance issued by the Office of the Dean containing a clear impression of the school's official dry seal and addressed to the student;
Letter from the School Registrar requesting the issuance of a Student Visa 9(f) to the applicant addressed to:
